Personal
Family Service Division (PFS) share a common mission: To help
older people avoid or resolve the questions, concerns and
difficulties which occur after age 60. Helping older people to
remain independent in their homes is a continuing central
purpose of the total program. To fulfill this mission, The
staff at Aging Partners Personal & Family Services call on
the capabilities and cooperation of all the human service
resources in the eight-county area of Butler, Fillmore,
Lancaster, Polk, Saline, Saunders, Seward and York.
Program Goals:
- To assure
the safety, well-being and maximum independence of older
individuals.
- To increase
the benefit older people derive from human services by
improving their access, understanding, and appropriate use
of those services.
- To
comprehensively address the needs of older individuals
through inter-agency cooperation and the active
involvement of friends and family.
- To actively
represent the needs of older people and develop new
answers to their concerns.
Information
& Assistance
The First
Service Information Desk is the starting point for obtaining
information, determining alternatives and arranging services
or referrals for individuals age
60+ and their families in an eight-county area. The First
Service counselors basic function is to connect people with
resources in the community, provide
information about a broad range of community services and
state or federal programs that they may not be aware of.
The First
Service desk is primarily staffed by Information Specialists
who are advocates happy to assist with sorting out questions
and concerns regarding the best care options and open the way
for service delivery. Call for help answering questions

Our First
Service counselors are available weekdays from 9 a.m. to 4
p.m., except for Wednesday. In order to accommodate weekly
staff training, service is available Wednesday from 12:30 p.m.
to 4 p.m. Walk-ins are welcomed, but appointments are
preferred
Care
Management & Social Work
Care Management
and Social Work Services provide professional assistance in
assessing needs, planning and coordinating services for
individuals age 60 and older. They help to provide support for
the more frail and vulnerable adults to manage life’s
everyday challenges.
Our care
managers are professional social workers and nurses who are
trained to assess the living situation of frail or vulnerable
older people and to find community services or resources to
help meet their care needs. Our care managers arrange a time
for a home visit to meet the client in their home environment,
providing a better perspective in determining the service
required. Care planning begins with a comprehensive assessment
of the elderly person's needs, abilities, circumstances and
support. From the assessment, a detailed plan for home care is
developed. With the elderly person's permission and
participation, Aging Partners Personal & Family Services
counselor arranges and coordinates the delivery of various
in-home services. The elderly person's needs and care plan are
reviewed regularly. Adjustments are made, as necessary, to
maintain his or her safety and quality of life at home on a
long-term basis.
Social work
service is determined to be more short term involvement with
the client. Our Social workers furnish the same professional
services as case management, but it is less intense and more
temporary in nature. Our social workers address difficult
changes, re-certification or clarification of applications and
work with other agencies on behalf of client.
